POSITION SUMMARY:
As the Patient Services Coordinator, you are responsible for coordinating and scheduling clinician care
for the patient. This role involves meticulous coordination, strong communication skills, and the ability
to manage scheduling complexities in a healthcare setting. It requires proficiency in using Axxess for
scheduling and coordination purposes, as well as effective relationship management with caregivers
and internal staff. The focus on patient care continuity and operational efficiency underscores the
importance of timely and accurate scheduling and coordination practices.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Scheduling Patient Care Period:
a. Coordinate and schedule new & existing patients as required & ensuring shifts are filled.
2. Confirming MD Orders Time Frames and Scheduling Correct Frequency:
a. Ensure MD orders are correctly scheduled and aligned with authorized frequencies in
Axxess.
3. Maintaining Relationships with PRN Pool:
a. Determine availability of PRN (as needed) caregivers.
b. Introduce and build professional relationships with caregivers.
i. Coordinate with AD & local recruiter on PRN & caregiver roster maintenance
4. Unassigned Shifts
a. Contact and coordinate with Home Health Aide Coordinators (HHAC) to fill unassigned
shifts.
b. Document communication notes in Axxess regarding shift filling.
5. 24/7 Staffing Assistance:
a. Provide staffing assistance around the clock as needed in conjunction with Travel Nurse
Program Manager.
6. Needs List
a. Manage and maintain needs list.
7. Orienting Staff to Patient:
a. Orient new staff members to patient in Axxess.
8. Maintain Availability in Axxess (Future State):
a. Ensure availability of staff is accurately maintained in Axxess.
9. Manages Missed Visits Tasks:
a. Handle tasks related to missed patient visits by caregiver through QA workflow
10. Follow-up on Not Started Visits
a. Coordinate with HHAC to follow up on visits that have not started.
b. For Skilled Nurse visits – on Monday send to Clinical Manager.
11. Maintain confidentiality of all office and client records per HIPAA and Privacy Act regulations.
12. Performs other duties as assigned.
OTHER DUTIES AND RESPONSIBILITIES:
This position may require occasional traveling between locations in your personal vehicle
SUPERVISORY RESPONSIBILITY:
This position has no supervisory responsibilities.
PREFERRED QUALIFICATIONS:
1. High school diploma or equivalent
2. Experience as a scheduler, or equivalent experience, preferably in the health- care field.
3. Excellent communication, and interpersonal skills
4. Ability to work as part of a team
5. Strong organizational and time-management skills
6. Proficiency in Microsoft Office Suite
7. Must pass a criminal background check and drug screen
8. Professional appearance required
WORK ENVIRONMENT and PHYSICAL REQUIREMENTS:
1. Ability to sit at a computer terminal for an extended period of time.
2. While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear,
and use hands and fingers to operate a computer and telephone, keyboard, reach, stoop and kneel.
3. Specific vision abilities required by this job include close vision requirements due to computer
work.
4. Regular, predictable attendance is required as business demands dictate
